Job Summary

Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Los Angeles, California. In this role, you will conduct PASRR Level II Pre-Admission Screening and Resident Review assessments to help determine appropriate placements and next levels of care for individuals with complex needs. As a Clinical Evaluator, you will conduct behavioral health assessments for the geriatric population as well as individuals with Intellectual Disabilities (ID), Serious Mental Illness (SMI), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations play a vital role in identifying the appropriate next level of care and supporting improved long-term health outcomes. 

 

This full-time role offers both flexibility and purpose combining remote work from your home office with local travel. Acentra Health provides reimbursement for travel-related expenses associated with this role, including mileage paid at the federal reimbursement rate and covered parking. We provide all essential technology and equipment needed for your home office setup, including a company-issued laptop for use both at home and in the field.


Job Responsibilities

  • Conduct PASRR Level II assessments by reviewing relevant records, gathering collateral information, and evaluating the individual’s physical, behavioral, and psychological functioning to support informed clinical decisions.
  • Responsible for conducting Preadmission Screening and Resident Review assessments and developing informed level-of-care recommendations, and determining the most appropriate next level of care for those who may require long-term care placement.
  • Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.
  • Provide well-informed, clinically sound Level of Care recommendations based on comprehensive evaluations.
  • Demonstrate strong knowledge of standards of practice for Level of Care determinations, as well as mental health, geriatrics, and other conditions commonly requiring long-term care placement.
  • Apply knowledge of standards of practice related to Level of Care determinations, as well as mental health, geriatrics, and other conditions commonly requiring long-term care placement.
  • Determine appropriate clinical diagnoses that account for psychological and behavioral factors.
  • Ensure compliance with required workflows, including re-reviewing records based on input from the psychiatrist.
  • Use computer equipment and web-based software to complete documentation and conduct work.
  • Understand, and comply with corporate policies, including those related to HIPAA and its Privacy and Security Rules.
Qualifications:

Required Qualifications and Experience

  • Must possess an active, unrestricted license in the State of California in a healthcare or related field, such as Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Psychologist (LCP), Licensed Mental Health Professional (LMHP) and Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • Graduation from an accredited college with a relevant degree program and a minimum of 2 + years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
  • Minimum 1 year of experience working with individuals with intellectual or developmental disabilities or related conditions.
  • Proven ability to assess and analyze psychological, emotional, behavioral, and physical functioning to support accurate and thorough clinical evaluations. 
  • Experience in medical record abstraction and clinical data review.
  • Strong communication skills, with the ability to interact effectively and with empathy with individuals and groups through verbal, written, and electronic communication.

Preferred Qualifications

  • Basic knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including compliance with state and federal regulations.

  • Familiarity with Federal Regulation 42 CFR, Chapter 483, regarding long-term care facilities and resident assessment requirements.

  • Knowledge of DSM psychiatric disorders and Level of Care standards, with experience diagnosing conditions using DSM criteria (California-specific).

  • Detail-oriented with excellent organizational skills and the ability to prioritize and complete tasks efficiently within established timelines.
  • Previous experience working with the geriatric population.
  • Proficiency with computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).

Compensation

The compensation for this role ranges from $85,280.00 to $87,360.00 annual base salary.

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
